Annual Reporting System

Researchers are required to periodically submit reports regarding the status of their projects (typically annually and in response to a request). Level 2 Protocols reviewed and approved by the IRB are the most likely to require a report (i.e. not Level 1 IRB exempted protocols).

The purpose of the report is to assure the IRB that all persons collecting data, obtaining consent, or having access to data that might include identifiers are appropriately trained and supervised. The report also indicates when Miami can archive inactive files.

There are two ways a report may be submitted: 1) using an online form; and 2) downloading an MS-Word form and returning the completed report by email attachment(s). The nature of your lab/protocol will dictate the most efficient method.

1) Online Form Annual Report

This is a link to an online Google Form. It would be wise to have a personnel roster updated and available before beginning the form. Personnel forms are available but the original submitted with the protocol or most recent update may be used as a more efficient starting point.

2) Email Attachment Submission

Selecting this link will download a zip file containing the form as well as blank personnel rosters. However, the updated original rosters may be submitted.
